To talk with ayano you go to clubroom and click her name. To recruit members, click on an npc the go to invitations > invite to occult club (talk to them a bit so that they're more likely to join).
in the dialog options for a random NPC, there will be an 'invitations' section where you will have the option to try and recruit a club member if you've gotten that far with Ayano. If you don't see this then try talking to the student council head. Now I don't remember exactly how that all plays but that should get you started in the right direction.
Bug in v932, after getting the club to five members, it continuously repeats the completion dialogue with Ayano and brings up an error. If you click ignore, it repeats the dialogue, and if you click reload, the game crashes. Rollback sometimes fixes this glitch, but other times will just leave you in a loop of the last piece of text and the error screen.

It already exists. This can only be done by randomized NPCs who have a high vulgarity... Actually, what else can you expect from them in a simplified character system? Also, even with this in mind, they won't do it if your character has enough relationship points with her - 100.
Anyway, if you have any problems with this, you can make randomized NPCs less vulgar using the point generation function. They do this with a vulgarity above 90, which already a priori has a small proportion of RNPCs in normal generation conditions.
That was actually the first thing I tried. I made an NPC with depravity set to zero, and who was as much of a tsundere as I could make to try to avoid the scene. It doesn't list anything but a blank for her current corruption, but as soon as her attachment was over 1000 she would blow a random guy on every text. The first time I saw this scene was a high vulgarity character, so I assumed the same thing that you suggested.
The relationship over 100 and they won't do it thing isn't correct. It seems to actually activate the scene when relationship is high enough. My recent test character has an attachment of 1250 and she does it every single time I text her my location.
In any case, if this isn't intended as cuck fetish content, there should at least be an option to do something about it other than watch like a massive cuck or tell them not to do it in public. Even better would be an option to just disable scenes like this entirely.
If you can, throw off your save with a given fact or a video recording with a similar bug. All the conditions are clearly spelled out in the call of this event, so if there is a problem, it is caused by the fact that RenPy ignores the conditions and incorrectly calculates everything.

I think in this case, however, the dev seems very engaged with feedback, and interested in making a good product. Once he makes an update, I'm going to give it another shot. Another commenter compared it to Artificial Academy, but I think the RPG elements this game has could give it a slight edge on AA as it gets more polished.
